Prescription Labels as it Relates to the MARs


No Group Home in my experience has the same MAR Sheet.

You must pay close attention to what the prescription states and how it was typed onto the MAR. If you are unsure of the directions or anything else on a prescription, refer to the original label on the prescription bottle/box.

The doctor or nurse should have explained what the medication  was for along with or during the diagnosis. Once the prescription was filled the Pharmacist offers consultation if the description of the medication was not given. Remember you can ask questions. Not every group home keep that part of the nurse's book in the home. Another option is to use you cell phone to look the medication up/Google.




Date Started - 6/27/07 should be recorded on the MAR


Side Effects can be found on the prescription receipt, pre written on the MAR depending on the company, or looked up on your phone or computer. If you are starting a shift and no "communication book" is put in place, or the company does not record side effects on the MAR. SCENARIO: A consumer tells you that their feet are swollen and it has not happened before this could be a side effect of the new medication. There should also be a procedure for this scenario. 

Now that you read about the label and what to look for. Let's talk about the MAR being a legal document. It used to record administration of medication. They are normally created and filled in by company Caseworker, a pharmacist, a company nurse or other specified. A nurse or professional MUST delegated staff on the medication listed and sign off on the MAR. MARs are normally given to group home staff monthly. Once the MAR is received in the home is still staff’s responsibility to check for errors. If an error is found follow your company’s policy. All medication prescribed by the doctor MUST be listed including over the counter medication.
