Community Awareness

Community Support- Community supports are individualized activities that are planned outside of the consumer's home in community locations, such as:

Library
Gym
Provider planned events or activities
Grocery Shopping
Hair cut/Nail Salon or related
Scheduled appointments such a the doctor
Other (outside the home)

Scheduled everyday activities such as work does not have to be recorded.


This service is documented on a “Community Awareness” Sheet, monthly to be turned in, depending on the Provider/company.
